The Judgment Calls a Scanner Cannot Make

What We Keep Finding
A scanner can prove your image has an alt attribute. It cannot tell you the text reads "IMG four zero three two" where a customer needed "brown leather boot". That gap runs the length of the standard. We graded 356 of W3C's 432 techniques and failures, and 10 can be settled by a machine outright. The rest need a person to look, read and judge, either because a tool finds some cases and not others, or because it cannot see the thing at all.
You do not have to take our grading for it, because the standards body publishes the same conclusion from the other side. W3C runs 87 live test rules, and on 59 of them a clean pass is recorded as meaning the criterion still needs further testing. Exactly four rules in the whole set can clear a criterion outright. Narrow it to the 55 rules an audit tests and 24 of them have no published test rule of any kind. That is the standards body saying, on every rule page it publishes, that passing automatically is not the same as conforming.
Manual review is just a formal way of saying human review.
