Every iframe is a page inside your page, with its own rules and its own failures. An untitled frame gives a screen reader user nothing to go on. Keyboard focus wanders into an embed and cannot find its way back out. The part that gets underestimated is what an embed does to the page around it. Two rules here do not care whose code it is. A keyboard trap inside a frame stops the entire page conforming, at any level. And where a frame sits inside a purchase or a booking, every page in that process has to work for the process to count, so a payment frame nobody can complete does not fail on its own. It takes your whole checkout with it.
Maps deserve a note of their own, because they attract findings that are not real. A map is allowed to scroll in two directions where a page is not. Its color gradients are allowed to fall below the contrast requirement where they carry a measurement. And a map that pans using the browser's own scrolling is outside the dragging rule entirely, while one that suppresses native scrolling and implements its own is inside it. We test to those exceptions rather than through them.

The Honest Limit
One honest limit, and one gap we found rather than invented. A third-party frame's internals belong to its vendor, so your page-side duties, meaning titles, focus handling and alternatives, are fully fixable and fully covered, while the vendor's behavior gets documented with evidence you can escalate. The gap is that no accessibility rule requires a fallback for an embed that is blocked or fails to load. We will still test it and still tell you, and we will mark it as good practice rather than as a failure, because a finding you cannot be held to should be labelled that way. And while you wait on a vendor, the standard gives you two positions rather than none. Monitor the embedded content and repair what breaks within two business days, and you can still claim conformance. Or publish a statement that the page would conform if the uncontrolled part were removed, naming that part clearly enough for a visitor to recognise it. What is not available is quietly leaving the widget out of the claim, because a claim covers the whole page or it covers nothing.
